Steps to take to ensure insurance coverage:
1) Call the number listed on your health insurance card
2) Ask the following questions:
- Are CPT codes 97802 and 97803 with ICD diagnosis code of Z71.3 covered?
- If diagnosis code Z71.3 is not covered, are specific diagnoses covered (Like a BMI code or an eating disorder diagnosis)?
-Does a deductible need to be met before coverage?
-Is a referral or prior authorization required?
-Is a copay needed at the time of visit? How much?
-Are there any limits on the number of visits covered in the plan?
